Extended Care Registration

Medical Forms for 22-23
- All Kindergarten students must have two varicella shots (chicken pox) and two MMR's (measels, mumps, and rubella) for the 2022-2023 school year.
- All students, please make sure that a current copy of your child's immunization form is sent to the nurse's office. The school's fax number is 410-663-9277. Please be sure to complete the medical forms by September 1, 2022. To view the health forms needed, please click here. For administration of medication at school, please complete this form.
- NEW - In order for a student to receive any over-the-counter medication, a completed Consent for Administration of Over-the-Counter Medications form must be on file. All over-the-counter medications must be brought into school by the parent or guardian and given to our school nurse. All medications must be labeled with the student's name in the original container.
